Demonstrates patient education and counseling through affecting behavioral change. Responsible for overseeing the day to day operations of the DSME program by ensuring that the National Standards (NSDSME) are met and maintained at all times. Oversees the planning, implementation, and evaluation of the DSME program. Arranges and coordinates the Annual Advisory Group. Monitors and maintains the organization of DSME program staff qualifications, including CE credits, licensures and registrations. Responsible for maintaining ADA Recognition and participates in the evaluations of the DSME Program’s effectiveness. Works with Healthcare team to maintain Evidence Based Diabetes Protocols and Policies for the hospitalized patient. Leads the implementation and education of staff of current and changes regarding Evidenced Based Diabetes Protocols. Diabetes Coordinator empowers the patient to implement care that the physician has prescribed and learn day-to-day lifestyle management. Volunteers to represent Labette Health at outside events several times a year to promote diabetes education. Does a follow-up with new diabetic inpatients prior to dismissal Works with case management and social services to provide needed supplies and resources for patients to maintain compliance (such as glucometer, testing strips, insulin)Look for opportunities to expand the Diabetes Programs at off site locations

successful completion of a prescribed curriculum of study which has led to licensure as a Registered Nurse in Kansas

  • Must possess the knowledge of the lifelong process of managing a chronic disease.
  • Clinical leadership, management experience or Clinical practice experience.
  • Completion of CDE certification process at soonest possible testing date.
