Summary:

• Provides COVID-19 screening to all individuals entering the facility; including taking temperatures and asking the screening questionnaire.

• Explains to individuals visiting the facility the need for screening; prevents visitors from entry into the facility if screening criteria is not met.

• Provide individuals entering the facility with a mask for those who do not have one upon entering the facility.
